To a CMI-qualified professional, a business is simply a "patient" made of capital, people, and processes. When you "plug in" your diagnostic tools, you are looking for Systemic Arrhythmia (inefficiency) or Internal Blockages (bottlenecks).Here is your template for a Consultant’s Clinical Case Note, designed to be used for your website articles or your independent consulting practice.

The Strategic Case Note: ( Business Name/Project)

1. Subjective: The Patients Complaint 

  • Chief Symptom: What does the owner think is wrong? (e.g., "We aren't making enough profit," or "Staff are leaving.")
  • Medical History: How long has this been happening? Is it a "chronic" issue or an "acute" failure following a change in the market?
  • Bedside Manner: Observe the "Body Language" of the office. Is the electrical system (morale) flickering?

2. Objective: The OBD Data Stream

  • Vital Signs (The Numbers): * Current Ratio:(Liquidity/Blood Volume).
    • Gross Margin: (Metabolic Efficiency).
    • Customer Acquisition Cost (CAC): (Energy required to grow).
  • Diagnostic Scan (Error Codes): * Code P0101 (Process Airflow):Bottlenecks in the supply chain.

3. Assessment: The Clinical Diagnosis

  • Root Cause Analysis: Using the "5 Whys"to move past the symptom.
    • Symptom: Low profit.
    • Diagnosis: High "mechanical friction" in the delivery process is wasting 20% of every pound spent.
  • Prognosis: If left untreated, the "Engine" (Cash Flow) will seize within 6 months.

4. Plan: The surgical Intervention

  • Immediate Triage (The Fix): Short-term actions to stop the "bleeding" (e.g., cutting "Legacy" costs).
  • Preventative Maintenance (The Service): New "Filters" (Standard Operating Procedures) to ensure the system stays clean.
